Job Description
The Case Manager (CM) will support the implementation of HIV prevention programs targeting at-risk adults and children within the district. The CM will work closely with health facilities and communities to ensure effective navigation of clients to ART initiation, retention in care, and re-engagement of those who have missed appointments or been Interrupted in Treatment (IIT). The role involves supervising and building the capacity of Peer Navigators and ensuring a client-centered approach throughout service delivery.

Specific Duties
  • Manage and execute Enhanced Linkage case management to achieve 95% enrolment in care and treatment services.
  • Implement and oversee tracking and retention strategies to maintain ART adherence rates.
  • Provide technical support to Peer Navigators to reduce quarterly net loss by 95% and document best practices.
  • Collaborate with peer navigators, healthcare workers, and facility partners to achieve 100% biometric registration coverage.
  • Ensure 95% linkage to care for newly diagnosed clients in both community and facility settings.
  • Support healthcare providers in delivering ART adherence counseling and enhanced counseling for clients with high viral load.
  • Mentor and support peer navigators and community-based health workers in linking PLHIV to care and proper documentation.
  • Provide intensive adherence support to patients with Advanced HIV Disease (AHD).
  • Assist in early identification and follow-up of missed appointments.
  • Promote the use of national follow-up tools and ensure proper documentation.
  • Facilitate regular meetings with Peer Navigators, healthcare workers, and other implementing partners to share challenges and develop improvement strategies.
  • Identify, enroll, and support clients in Community ART, optimizing adherence.
  • Conduct daily follow-ups on case management activities with the team.
  • Reconcile and compile data into regional tracking registers and partner outcome listings weekly.
  • Conduct site visits for at least 70% of working time.
  • Work with the Data Manager/M&E Officer to report tracking and partner listing outcomes to the MER team.
  • Prepare weekly and monthly reports on tracking services, partner notification, and success stories.
  • Participate in CSO-level and CHMT meetings.
  • Document best practices for linkage of Key and Vulnerable Populations (KVP).
  • Perform other duties as assigned by the line manager.
Qualifications and Experience
  • Bachelor’s Degree or Diploma in Nursing, Social Work, Counseling, Psychology, or related discipline.
  • At least 3 years of experience in biomedical HIV prevention, including HIV care and treatment, HIV counseling and testing, and Sexual Reproductive Health (SRH).
  • Proficiency in spoken and written English.
  • Experience working with high-risk and vulnerable populations.
  • Previous experience in USAID/PEPFAR-supported HIV prevention programs is an added advantage.
